Linking style

How many conditions can one medication support?

One-to-oneEach medication supports at most one condition — the most specific match wins. One-to-manyA medication can support every selected condition it treats. FamilyA medication supports one condition family; all selected conditions in that family are covered. Model-based (mixed)A medication may support any number of RxHCC conditions, but at most one medical-model (V24/V28) HCC — the most specific wins the slot. Matches plans that allow one-to-many for Rx but not across medical HCCs.

Full captureShow every condition relationship, HCC or not. Medical HCC only (V24/V28)Only treat relationships for conditions that risk-adjust in the CMS-HCC medical model. RxHCC only (Part D)Only conditions that risk-adjust in the RxHCC model — includes asthma, hypertension, lipids, thyroid, osteoporosis, migraine, BPH, glaucoma. Any HCC (medical or Rx)Conditions that risk-adjust in either model.
